Abstract
Objectives: In chronic diseases such as rheumatoid arthritis (RA), carbamylation may cause the production of autoantibodies such as the anti-carbamylated protein antibody (anti-CarP) and protein malfunctioning. However, its relation to periodontitis (PD) remains unclear. Therefore, this study aimed to assess the serum levels of anti-carbamylated protein antibodies (anti-CarP) in four groups of participants, the RAPD, RA, PD, and healthy groups and correlate the findings with rheumatological parameters. Materials and methods: This pilot study included a total of 20 participants, with 5 participants each in groups diagnosed with RA and PD (RAPD), only RA, only PD and controls who were free from both diseases (HC). A basic periodontal examination (BPE) was performed to group participants into PD and non-PD groups. Ten ml of venous blood sample was collected from all participants and serum was separated via centrifugation. Quantification of anti-CarP was performed using enzyme-linked immunosorbent assay (ELISA). The RA disease duration and DAS28-ESR scores were also recorded. Results: The highest mean for serum anti-CarP levels were found in those in the PD group (42.08±11.064 ng/mL) followed by the RA group (35.50±24.54 ng/mL), healthy controls (31.86±9.18 ng/mL) and the lowest in the RAPD group 14.20±7.20 ng/mL). The serum anti-CarP levels were statistically significantly higher in participants with PD compared to those in the RAPD group (p=0.021). Bivariate correlation analysis revealed a weak correlation with no statistically significant difference between the serum anti-CarP levels and the DAS28-ESR (Spear p=0.092; p=0.8). iv Conclusion: The circulating anti-CarP levels may be strongly associated with PD but not with RA in this pilot study. However, this remains inconclusive as further investigations involving a larger sample size are needed to verify the association.
